You will need the following information to complete the registration process:

· Birth certificate (Your child must turn 5 on or before September 30)

· Current shot record

· Child’s social security card

· Parent or Guardian’s driver’s license

· Two utility bills (water, gas, electric, lease agreement, mortgage payment, utility bills) including physical address for proof of residence (no PO Box numbers as proof of residence).  A cell phone bill is not acceptable.
